The preferred methods for applying are the Online
Application or the Print From Web
Application. Compare all three ways to apply
to choose the correct method for you.
If you are unable to use either of these, you can contact the Department
and request to have an application package mailed to you.
The application package contains all of the same materials, and requires
the same information, as the other methods. All instructions are contained in
the package.
You can call, fax, email, or mail your request for an application package.
Be sure and provide your COMPLETE name and mailing address. Send request to:
